Case Manager - Home Healthcare / Remote Position

We are seeking a highly organized and proactive Case Manager to join our growing home healthcare team. This is a fully remote opportunity for someone who thrives in a fast-paced environment and enjoys coordinating schedules while ensuring excellent operational support. Key Responsibilities:
- Coordinate patient and caregiver schedules to ensure appropriate staffing coverage.
- Manage daily scheduling needs and respond efficiently to last-minute changes.
- Resolve staffing gaps and scheduling conflicts.
- Support the day-to-day operations of a home healthcare agency.
- Maintain professional communication with patients, caregivers, and internal team members.
- Previous experience in healthcare coordination, scheduling, dispatch, or administrative support is preferred.
- Strong organizational, multitasking, and problem-solving skills.
- Ability to adapt quickly to changing priorities and urgent scheduling needs.
- Excellent English communication skills with a neutral accent.
- Bilingual in English and Spanish (required).
- Preference will be given to candidates based in the Dominican Republic.
- Saturday & Sunday: 9:00 AM – 5:00 PM EST
- Monday – Thursday: 5:00 PM – 12:00 AM EST
