Case Manager

We are seeking a compassionate, organized, and detail-oriented Case Manager   to support clients by coordinating services, developing care or service plans, and ensuring that individual needs are met efficiently and effectively. The ideal candidate will have strong interpersonal skills, the ability to multitask, and a commitment to helping others navigate resources and support systems.

This is an Hybrid POSITION- We are located at the Coral Mall SDE

This position requires a C1 English 

• Assess client needs and develop individualized case or service plans
  

• Coordinate services and resources across departments, agencies, or providers
  

• Maintain accurate and timely documentation of client progress and case updates
  

• Serve as the main point of contact for clients, families, and partner organizations
  

• Monitor client progress and adjust plans as needed to achieve outcomes
  

• Advocate on behalf of clients to ensure access to services and support
  

• Ensure compliance with internal policies, legal requirements, and industry standards
  

• Conduct regular follow-ups to ensure continuity of care or services
  

• Participate in team meetings, case reviews, and training session  
  

Requirements

• Hands on experience on Social Work, Psychology, Healthcare, Human Services, etc.
  

• 2+ years of case management or related experience
  

• Strong organizational and time management skills
  

• Excellent verbal and written communication abilities
  

• Ability to handle sensitive information with professionalism and confidentiality
  

• Proficiency in case management software and Microsoft Office Suite
  

• C1 English Level 
  

• Certification in case management (CCM, ACM, or equivalent)
  

• Experience working with CRM platforms 
  

• Crisis management or conflict resolution training
  

• Knowledge of local resources, support services, or legal/healthcare systems
